Job Description
The On Call Hospice LPN plays a critical role in providing compassionate and timely care to hospice patients. This position involves managing patient symptoms, administering medications, coordinating care, and supporting both patients and their families during end-of-life care. The On Call Hospice LPN must be prepared to respond to emergencies and maintain thorough documentation of patient status and care delivered. On Call Nurse will be on call every other week. Monday through Thursday 5:00 pm to 8:30am, Friday 5:00pm to Monday morning 8:30am.
Responsibilities
- Deliver direct patient care focused on comfort and symptom relief
- Administer medications according to prescribed plans
- Monitor and manage patient symptoms to enhance quality of life
- Coordinate care activities with interdisciplinary hospice team members
- Provide emotional and informational support to patients’ families
- Document care activities and patient responses accurately
- Respond swiftly and effectively to emergency situations
- Perform comprehensive patient assessments regularly
- Maintain clear and ongoing communication with healthcare team and families
Preferred Qualifications
- Minimum of 1 year experience in hospice nursing
