Position: Home Health Occupational Therapist

In this role, your responsibilities will include:

  • Evaluating and screening patients’ daily living and work-related skills, while creating therapeutic retraining programs with measurable goals.
  • Administering and interpreting diagnostic and prognostic tests to assess patients’ functional abilities in their homes or other environments, aiding physicians in evaluating their level of function.
  • Collaborating with the patient’s physician and other healthcare team members to develop and revise the Plan of Care Treatment.
  • Delivering therapeutic treatments and guidance to patients as per physician directives to enhance or restore strength, coordination, range of motion, and overall function, or teaching compensatory strategies.
  • Reviewing and expanding retraining programs in alignment with each patient’s behavioral goals and the overall rehabilitation program guidelines.
  • Consulting with vocational team members, as necessary, to create and implement vocationally oriented plans that align with patients’ needs and capabilities.
  • Reporting on patient status and progress to the physician, rehabilitation staff, Clinical Manager, and family members.
  • Engaging in care coordination and discharge planning activities, serving as a resource for other healthcare personnel to meet patient needs.
  • Designing community reintegration activities to support clients in their physical reconditioning and psychological adjustment, coordinating with the interdisciplinary team.
  • Recommending or designing specialized adaptive equipment to enhance clients’ home or work environments and facilitate their participation in rehabilitation and community activities.
  • Training staff and family members to implement the retraining program, including skills related to dressing, feeding, grooming, and hygiene, and participating in educational meetings with the team and family.
  • Supervising, instructing, and evaluating the performance of Occupational Therapy Assistants and Home Health Aides in delivering occupational therapy services.
  • Documenting patient care observations, interventions, and evaluations accurately and thoroughly on the day services are provided.
  • Ensuring that interim (verbal) orders from the physician are documented and executed correctly.
  • Submitting evaluations, treatment plans, and discharge summaries to supervisors and care management staff.

Utilize Your Skills to Make a DifferenceRequired Qualifications:

  • Current and unrestricted Occupational Therapy licensure.
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation.

Preferred Qualifications:

  • At least six months of experience in occupational therapy.
  • Experience in home health settings.

Compensation

Pay Range: $54.00 – $76.00 per visit/unit, with an annual base salary between $85,400 and $117,500.

Additional Information

TB Statement: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will be required to undergo TB screening.

Driving Statement: This role is included in our driver safety program, necessitating a valid state driver’s license and personal vehicle liability insurance. Individuals must maintain vehicle insurance that meets or exceeds state minimum requirements, or $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 for property damage, whichever is higher.

Scheduled Weekly Hours

The compensation range provided reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, knowledge, experience, education, and certifications.
