Our organization is currently looking for a full-time Primary Care Nurse Practitioner or Physician Assistant to become a vital part of our clinic located in the scenic Daytona Beach, FL.

As a Nurse Practitioner (NP) or Physician Assistant (PA) within our Senior Primary Care team, you will collaborate with a diverse group of professionals, including physicians, pharmacists, care coaches, and behavioral health specialists, all dedicated to providing tailored, high-quality healthcare.

You will report directly to the Associate Medical Director.

Your Responsibilities

  • Deliver comprehensive care to senior patients in accordance with clinical standards.
  • Engage daily with your care team to ensure seamless, high-quality service.
  • Oversee referrals, coordinate with hospitals/SNF, and manage DME and home health services.
  • Document patient care with assistance from quality-focused coders.
  • Participate in on-call rotations and contribute to growth initiatives at your center.
  • Have experience in providing value-based care.
  • Work on-site at the clinic.

What We Are Seeking

Essential Qualifications:

  • Master’s degree in nursing or completion of a PA program with board certification.
  • Active and unrestricted NP or PA license in the state where you will practice.
  • Experience in value-based care, primary care, or Medicare Advantage environments.
  • Possession of an active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
  • Familiarity with HEDIS quality indicators and Medicare regulations.

Scheduled Weekly Hours

40 hours per week

Compensation Range

The salary range provided below represents a good faith estimate of the starting base pay for full-time employment (40 hours per week) at the time of this posting. Actual compensation may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

$123,800 – $170,300 annually

This position is also eligible for a bonus incentive plan based on individual and/or company performance.
