FULL TIME LICENSED PRACTICAL NURSE (LPN)

Job Description:

• Performing skilled nursing services including: wound care, lab work, patient education, etc.
• Monitoring and recording patient’s vital signs as needed; reporting abnormal readings to the supervisor
• Observing patient’s functional level; reporting any changes in the member’s physical condition and health care needs to the supervisor
• Assisting patients, as needed, with medications; observing for possible reactions; reporting any reactions to the supervisor
• Completing an in-home supervisory visit with assigned members at least once per year to observe and monitor the in-home performance of the personal support aides
• Preparing progress notes and supervisory visit entries, documenting patient’s current status
• Collaborating with the supervising RN to update patient care plans

Job Qualifications:

• Must be willing to work in patient homes
• Graduate of an approved Professional Nursing Program.
• Current LPN license, with no sanctions, restrictions, or provisions, in the state of Georgia.
• One year of related experience in a clinical environment
