This role is based in our clinic, operating Monday through Friday, with weekends off!

Position Overview and Qualifications

Overview:

The Licensed Practical Nurse (LPN) in the PACE program is crucial in assisting our participants through thorough medication management and team collaboration. Following the PACE care model, which prioritizes an interdisciplinary and participant-focused approach, the LPN helps individuals maintain their independence and safely reside in their homes and communities. This role involves ensuring accurate medication administration and documentation, monitoring changes in patient conditions, and providing direct care. Working alongside providers, pharmacists, caregivers, and the interdisciplinary team, the LPN facilitates communication, care coordination, and continuity of services to enhance participant outcomes and improve quality of life.

Required Qualifications:

Key Responsibilities:

  • Deliver direct patient care in an outpatient environment, assessing outcomes and modifying nursing processes to comply with legal, ethical, and clinical standards.
  • Performance Expectations:
  • Monitor and document patient vital signs.
  • Assist providers with therapeutic and diagnostic procedures.
  • Administer prescribed oral and injectable medications, observing patient responses.
  • Delegate tasks to licensed and unlicensed staff based on their competencies and scope of practice.
  • Provide exceptional customer service to patients and their families.
  • Triaging participants to address their needs and concerns, making clinical judgments regarding the necessity of physician visits based on symptoms.
  • Coordinate care with specialist providers and nursing facilities as needed.
  • Collaborate with the care team to implement clinical goals and interventions.
  • Assess participants’ medical needs and develop care plans to support their success in the community.
  • Coordinate care orders and interventions as prescribed with skilled nursing facility partners.
  • Engage with patients, colleagues, and the interdisciplinary team to achieve desired health outcomes.

Additional Performance Expectations:

  • Consistently document and report patient data, responses, and progress.
  • Tailor patient education and care plans to meet individual needs.
  • Advocate for patients and families to help achieve desired health outcomes.
  • Identify challenges and implement solutions to meet the needs of patients and families throughout their care journey.

Qualifications:

  • Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) certification must be obtained within 2 years of starting employment.
  • Associate Degree is required.

No travel is required for this position.
